Birth/death records

Effective Tuesday, March 9, 2021 the City of Lakewood Division of Vital Statistics is temporarily suspending in-person purchases of birth and death records.

Birth and death records can be obtained by calling (216) 529-7690 Monday through Friday from 8:00 a.m. to 4:00 p.m.

Funeral directors are encouraged to call for an appointment.

A fillable PDF application to request a certified birth or death certificate is available and can be mailed, faxed, or e-mailed to the Division.  Please note that a required fee of $25.00 is charged for every copy of birth and death records certified.  Further information is below.

REQUESTING BIRTH AND DEATH RECORDS:

Though the Division of Vital Statistics is currently not accepting walk-in requests, the Division can still make a certified copy of a birth certificate of any citizen who was born in Ohio. The Division can also make a certified copy of a death certificate of any citizen who died in the City of Lakewood.

To obtain these certificates, download our fillable application online and return the completed copy by mail, e-mail, or fax.  NOTE: You MUST open the application in Adobe Reader for the fill-in fields to work properly.
