LAKEWOOD CERTIFIED BIRTH AND DEATH RECORDS |
| Certified birth and death records
can be obtained one of four ways for births and deaths that
occurred in Lakewood only. |

In person: visit the office
during regular business hours. Complete a short form and the
certificate will be retrieved, copied, and certified for a fee
of $20.00 per certified copy. |

By mail: Send a written request
to the office that includes the name of the individual, date
of birth or death, parents’ names (birth certificates only)
and a daytime telephone number. Include a self-addressed stamped
envelope and a personal check or money order for $20.00 per
certified copy. |

By phone: Call the office
during regular business hours and state that you would like
to use a credit card. Mastercard, VISA, American Express and
Discover Cards are accepted. Provide information as requested
by the clerk. The credit card service, Vitalchek, charges a
$7.00 fee in addition to the fee of $20.00 per certified copy.
Overnight delivery is available upon request at an additional
charge. Certified records are sent via regular mail unless otherwise
specified. |

| Burial Permits are issued to funeral homes for
a fee of $3.00 each. |
