Social Work Services

To assist an older adult in independent functioning in the home, the social workers:

  • Conduct an in-home assessment of functioning and needs
  • Design a care plan with the resident/family
  • Monitor the quality of services provided

Social workers also provide assistance with:

  • Long-term care planning
  • information on health care resources
  • housing information for older adults in Lakewood
Seasonal Programs
  • Leaf Raking

    A program jointly sponsored by Divisions of Aging and Youth for eligible Lakewood homeowners. On a designated Saturday in November student volunteers rake leaves for elderly and/or disabled residents. Application process required.

  • Change Your Clock, Change Your Battery

    A program in cooperation with Lakewood Division of Fire designed to encourage older adults to change smoke detector and flashlight batteries when re-setting clocks from daylight savings time. On the designated Saturday for changing clocks, Firefighters visit seniors and change batteries in their smoke detectors. Application process required.
